Moving forward with a new administration

I want to preface this week’s newsletter by acknowledging a departure from my usual approach to healthcare. This edition takes on a more politically oriented perspective. While I typically avoid political topics—primarily because they are inherently controversial and seldom lead to definitive solutions—I feel compelled to address certain truths. These truths, extensively known to only a select few, reveal how financial interests have overshadowed the importance of the health of humanity and the sustainability of our planet.

For my regular readers, you may recall my discussion on President-elect Donald Trump's nomination of Robert F. Kennedy Jr. as Health and Human Services Director last month. What concerns me most about Kennedy’s plans is the sweeping nature of his proposed agenda, which includes significant overhauls to healthcare policies, including his stance on vaccinations and the removal of fluoride from the water supply—an undertaking of massive scope and complexity by itself.

In addition, he wants to reduce the prevalence of ultra-processed and “Junk” food in the Western diet. Perhaps many of you do not recognize what that would require. I have repeatedly stressed the fact that the involved industries responsible for production of vaccines, general healthcare, and food production (both supply and processing) is associated with an immense amount of national wealth.

To give you an idea of the size of this undertaking, I have compiled a list of the obvious involved elements in our economy. I have used AI to arrive at a reasonable estimate. 1. Profits from Vaccine Production and Distribution

  • Profit Margins: Major pharmaceutical companies (e.g., Pfizer, Moderna) report high margins, particularly for COVID-19 vaccines. For instance:

    • Pfizer's COVID-19 vaccine revenue reached $37 billion in 2021, with significant profits contributing to its overall margin of 27%-30%.

  • Non-COVID Vaccines: Profit margins for routine vaccines are typically lower, averaging around 10%-20%, depending on the manufacturer.

 

2. Cost of National Water Fluoridation (U.S.)

  • Annual Cost: Fluoridation of public water supplies in the U.S. costs approximately $0.50 to $3 per person annually. For the U.S. population (~330 million), the total cost ranges from $165 million to $1 billion annually, depending on local factors like infrastructure and fluoridation methods.

  • Cost-Benefit Analysis: Studies suggest every $1 spent on fluoridation saves about $38 in dental treatment costs, making it a cost-effective public health measure.

 

3. Profits of the Food Industry (Big Food, Agriculture, and Meat Production)

  • Global Agriculture Market: Valued at over $12 trillion annually (2021), with growth driven by increasing food demand and technological advances.

  • Big Food Companies: Companies like Nestlé, PepsiCo, and Tyson Foods report combined annual revenues exceeding $1 trillion, with profit margins of 10%-15% for most major firms.

  • Meat Industry (Global): Valued at approximately $1.4 trillion annually, with major producers like JBS, Tyson Foods, and Cargill generating significant profits.

  •  U.S. Meat Industry: Contributes about $250 billion annually to the economy, with steady profit margins.

Kennedy has promised to take a serious look at those who work for HHS and its agencies, including the Food and Drug Administration, the National Institutes of Health and the Centers for Disease Control and Prevention.

He said he wants to fire 600 employees at the National Institutes of Health, which oversees vaccine research, and replace them with 600 new employees.

“We need to act fast, we want to have those people in place on Jan. 20, so that on Jan. 21, 600 people are going to walk into offices at NIH and 600 people are going to leave,” Kennedy said, according to a video of his remarks posted on YouTube.

He says the public health establishment is too focused on infectious diseases and wants to redirect resources towards the chronic disease epidemic, including obesity, diabetes, autism and mental illnesses. He blames them on greedy corporations including drug companies that worry healthy Americans would be bad for their bottom line and food producers using harmful pesticides and additives.

Certainly, he is partly correct, so much of the healthcare industry is controlled by money… Instead of concern for what is best for humanity. Often these two forces are in direct conflict with each other. Is this another example of the “Golden Rule” where the one who has the most money RULES? In most cases, the answer is YES.

Presenting a more REALISTIC Approach

The Future of Healthcare

While I’m in general agreement with RFK Jr’s beliefs, they are not an accurate or complete assessment of the problems we face as a nation. In addition, RFK Jr. has gone on record making outlandish claims falsely accusing vaccine research of being the cause of diseases like HIV, Spanish Flu, and Lyme Disease!?

Making unsubstantiated claims like these, have prompted more than 75 Nobel Laureates to draft a letter to congress opposing his nomination stating “In view of his record, placing Mr. Kennedy in charge of DHHS would put the public's health in jeopardy"…such a letter from this source directed to congress has never been issued before…

For the moment, please allow me to focus on what I believe is the single most urgent need to correct if we hope to improve the health of our nation, and the world. On this issue, there is overwhelming agreement among scientists, and health professionals alike. WE MUST IMPROVE OUR DIET!

While it is true and evident that the industry I’ll refer to as 'Big Food'—which includes food producers and processors of both animal and agricultural products—employs every available strategy to increase yield, maximize profits, and reduce costs. However, these practices often come at the expense of nutritional quality and may introduce harmful substances, whether through genetically modified plants, their use in animal feed, or the application of pesticides to boost crop yields.

Knowing these facts, it is easy to build a case against ‘Big Food’ and accuse them of being “greedy”. But this assessment isn’t entirely fair, because it does not consider the fact that it is human preferences that are responsible for the creation of these “addictive” food choices. Can you entirely blame food producers for providing what people demand?

Equally important is the consideration of cost…we are not all wealthy. Sometimes we are forced to choose less expensive alternatives or do without, even when we know the quality of the food we eat is not the best available… Stop for a moment and recall that less than 100 years ago we experienced a severe economic depression in this country, that forced our ancestors to literally eat garbage… And for much of the world this is still true today, just look at Syria for example.

Here’s another dose of reality in a different sector…to give you a better understanding of the enormity of the problems we face. The FDA has the authority to recall dangerous and risky medical devices, but it has rarely done so, largely relying on manufacturers to take necessary actions when a product defect or spike in serious adverse incidents occurs. 

Since Congress passed the medical device amendments to the Food, Drug & Cosmetics Act in 1976, the agency has used its 518(e) mandatory recall authority just a handful of times. In 1991, the FDA ordered Medline to recall its “Dynafeed” enteral pump due to design flaws and failure to comply with good manufacturing practices. The agency issued three mandatory recall notices the following year and in 2008, recalled Nebion’s MRI device, citing lack of FDA approval and safety and effectiveness data, as well as noncompliance with GMPs.

The Food and Drug Administration (FDA) has the responsibility to protect the health of U.S. consumers by ensuring food, drugs and medical devices are safe. The FDA also has a mission to advance the health of U.S consumers by speeding innovations of such products. The latter is the reason the Public Citizen’s Health Research Group, the National Center for Health Research and a former FDA attorney are questioning whether the agency is fulfilling the former.

But is the FDA simply the proverbial fox guarding the hen house? AND...do you know how many former FDA employees go to work for big Pharma? This looks like an incestuous relationship, nepotism at the very least…

  • BMJ (British Medical Journal) study

In a 2016 study, researchers found that 15 out of 26 FDA reviewers who left the agency in the hematology-oncology division went on to work for or consult with the pharmaceutical industry. 

  • Science magazine investigation

In 2018, Science magazine reported that 11 out of 16 FDA medical examiners who left the agency after working on drug approvals went on to work for or consult with the companies they regulated. 

  • FDA commissioners

Nine out of the last 10 FDA commissioners have gone on to work for pharmaceutical companies. For example, Mark McClellan, who served as FDA Commissioner from 2002–2004, is now a member of the board at Johnson & Johnson. 

  • FDA reviewers

Research published in The BMJ found that more than a quarter of FDA employees who approved cancer and hematology drugs from 2001–2010 left the agency and went on to work for or consult with pharmaceutical companies. 

The term "revolving door" is used to describe the process of people moving between the pharmaceutical industry and the federal government. This can create conflicting interests that hinder the development of substantial improvement to our healthcare system by focusing on job security and maintenance of personal wealth. 

Nine out of the last 10 FDA commissioners have gone on to work for pharmaceutical companies. For example, Mark McClellan, who served as FDA Commissioner from 2002–2004, is now a member of the board at Johnson & Johnson. 

I submit to you that this situation can be corrected with the passage of a law or laws prohibiting the large-scale migration from the FDA to big Pharma and vice versa. Trump certainly has the power to do this, particularly if he has a Republican Congress… Trust me, such a law would not be popular with the FDA or big Pharma, but sometimes this is the price that must be paid for progress. More importantly, achieving this objective in this manner will result in far less conflict, chaos, and danger directed towards any one group or individual.

I believe that in a similar fashion, much of the pathogenic nature of processed and junk food can be improved by laws restricting the use of certain seed oils, elimination of highly processed grains as found in breakfast cereals, limiting the addition of unnecessary sugar and fructose to foods, altering the composition of sweet, carbonated soft drinks to substantially reduce sugar and fructose content. Just these changes alone would significantly reduce the damage done by these foods to our bodies, particularly for our children.

I can assure you there would be massive pressure applied to congress by lobbyists for special interest groups. In addition, most of our population would react negatively to these enforced changes, but so long as this is done slowly and carefully, they will accept it—In much the same way as our children react to enforced food rules…AND slowly but surely, we will begin to see the positive health effects brought on by these changes…and this world will be better as a result.

 Additional Initiatives that Could Improve our Healthcare System and save Money

  • Telehealth

Telehealth can be used to administer and support patient health through videoconferencing, streaming media, and cellular communication. Telehealth has become more prevalent since the start of COVID-19 and is expected to remain a key part of the healthcare industry. 

  • Electronic health records (EHRs)

EHRs are digital records of a patient's health history that can improve continuity of care and patient outcomes. EHRs help doctors diagnose and treat patients based on a holistic picture of their health. 

  • Remote monitoring

Remote monitoring devices, such as blood pressure cuffs or pulsometers, can transmit vital signs and other data to physicians. This allows physicians to make treatment changes. 

  • Preventative care

Investing in preventative care, early intervention, and proactive treatment of chronic disorders can help lower healthcare expenses. 

  • Improved communication

Good communication between patients and providers can help patients feel heard and ensure they understand their diagnoses and care plan. 

  • Single information system

Creating a single, unified information system can support clinical care, performance improvement, choice, and accountability. 

  • Empathy and understanding

Care that is empathetic and understanding can create a better experience for patients. 

Integrated use of Artificial Intelligence to Improve Healthcare

I am reluctant to continue to recommend the use of AI in healthcare because of the prevailing fear and uncertainty surrounding Artificial Intelligence at this moment in time.

I understand this concern. We must focus on controlling the ways in which it is used… However, “Pandora’s Box” has already been opened… we must confront this reality and do everything in our power to insure that is used for GOOD.

Understand this: the inherent danger of AI does not come from a machine; it comes from the humans that use it. We must determine the way it is used.  I have compiled a few logical and important ways that we can utilize this technology.

  • Personalized medicine: AI can tailor treatments to a patient's genetic makeup and health history. 

  • Drug development: AI can help researchers identify patterns in medicines to track drug development. 

  • Improved patient safety: AI can reduce errors in medical records and documentation. 

  • Better patient outcomes: AI can help patients manage their conditions by providing treatment recommendations and lifestyle changes. 

  • Improved quality of life: AI can help patients manage their conditions, which can lead to a better quality of life. 

  • Reduced healthcare costs: AI can help patients manage their conditions, which can lead to reduced healthcare costs. 

  • Improved health monitoring: AI can help monitor patients' health data over time and provide alerts in case of problems. 

  • Easier data sharing: AI can help providers keep track of patient data more efficiently. 

  • Disease detection: AI can help detect and track infectious diseases. 

  • Surgical training: AI can improve surgical training and procedures. 

Some challenges to the integration of AI in healthcare include data quality, privacy, bias, and the need for human expertise. 

In conclusion, I would like to leave you with the following thoughts… I believe we are at a critical time in the earth’s history. Humanity has come so far in the past 10,000 years. Sometimes it is hard to gain the proper perspective of the evolution of the earth and human civilization as a result of the short lifespan of our species.

Now imagine that if this period were represented in a graphic of a 12-hour clock, that the relative time that has elapsed in the past 10,000 years, which is the extent of the known history of humanity… would be equivalent to approximately 0.0000951 seconds!

Does this give you a better perspective of our place in history? At the same time, you should remember that 99% of ALL living species that ever existed, have gone EXTINCT! Knowing this, can you better understand the importance of correcting the recent chronic health problems humanity is currently experiencing?

At the same time, we must consider the health of our planet and other lifeforms that inhabit the earth. As the dominant species, we are responsible for much of the environmental degradation resulting from our technological and sociological development.

We must take responsibility for our actions and apply our very best adaptive abilities to begin addressing and correcting the damage we are doing to ourselves and the environment. I believe we are capable of this enormous task. However, to achieve success, we MUST work together as ONE to Force these changes that can transform the status quo from a philosophy of “personal gain” to one of ONE WORLD…ONE HEALTH…

I truly believe that we can achieve this goal, but realistically it is unlikely to occur in the near future because we a too focused on our immediate needs and desires as we have been since the time of the establishment of our species. Why is it that a catastrophe is usually necessary to unify us? Is war or an alien invasion necessary to bring us to our senses when the signs are all around us? You wouldn’t think so, but a more realistic answer is YES.

One thing is certain…massive and meaningful change does not happen overnight. It takes time…as it always does for us to realize that no one man, or political philosophy can achieve this alone. This can only occur through education, awareness, and respect for everything that surrounds us…
